This privacy notice explains how Leicester Coil & Implant Service, an NHS service provided by Willows Healthcare, uses the personal information you give us when you request an appointment.

Who we are

Leicester Coil & Implant Service is provided by Willows Healthcare, an NHS primary care provider in Leicester. Willows Healthcare is the data controller for the personal information collected through this website.

What information we collect

When you complete the appointment request form we collect:

  • your full name
  • date of birth
  • mobile number and email address
  • postcode
  • the service you are interested in
  • basic suitability answers (pregnancy, symptoms, age, urgent symptoms)
  • preferred day and time, and any access notes you give us
  • your consent to be contacted and to this privacy notice

We do not knowingly collect any other special category health data through this form. Please do not include sensitive medical detail in the free-text box — our team will contact you to discuss clinical information securely.

Why we use your information and our lawful basis

We use your information to review your request, check the appointment is clinically suitable, contact you, and arrange NHS care.

Under the UK GDPR our lawful bases are:

  • Article 6(1)(e) — performance of a task carried out in the public interest (the provision of NHS healthcare)
  • Article 9(2)(h) — processing necessary for the provision of health or social care

Who we share it with

  • Clinical and administrative staff at Willows Healthcare
  • Your registered GP practice, where relevant to your care
  • NHS commissioners and partner NHS organisations where required
  • Our IT and hosting providers, who process data on our behalf under written contracts (including Microsoft Azure, which hosts our intake service in the UK)
  • Google Analytics, only if you accept analytics cookies, to help us understand how the site is used. IP addresses are anonymised.

We do not sell your information and we do not use it for marketing.

How long we keep it

Information related to your NHS care is retained in line with the NHS Records Management Code of Practice. Appointment request data that does not result in care is kept for the minimum period necessary and then securely deleted.

How we protect your information

Your form submission is sent over an encrypted (HTTPS) connection to a secure NHS-approved processing service hosted in the UK. Access is restricted to authorised staff.

Your rights

Under the UK GDPR you have the right to:

  • be informed about how we use your data
  • access a copy of your data
  • have inaccurate data corrected
  • have your data erased, where the law allows
  • restrict or object to processing, where the law allows
  • data portability, where the law allows

Some of these rights are limited where we hold information as part of your NHS health record.
